Camp Forms
Wilson Collegiate Tennis Camps / Premier Sports Camps require a couple of forms to complete the registration process.
Required forms for all of our camps are the appropriate
1. Medical Form
2. Liability Waiver
3. Camp Information Sheet (Below)
In addition, we recommend that each camper downloads the Camp Information form. This form provides you with all the appropriate check in info, what to bring, locations etc…
* A note on the Medical Form – We require each camper to have a completed medical form prior to participation in our camp. This form should be completed and signed by your physician. Our form on this site is the desired form, however, we will accept a State medical form that each school requires for their participating student athletes, as long as the physical has been completed within 1 year of the start of camp.
** Liability Forms are required to be sent to the office prior to arrival. Please bring all other camp forms to check-in at the start of camp.
